Elder Care Homecare is seeking a New York State Registered Nurse to join our company's team in Manhattan. We are a private pay homecare agency servicing Westchester County, Long Island, and the NY Metro Area, with additional expansion planned within the next year.
Elder Care has been in operation for over seven years, however our growth has dramatically increased over the last two. This role is based in our NYC office.
As a Registered Nurse Care Manager, you will be responsible for supporting our clients and caregivers in the NYC area. The ideal candidate be able to effectively build relationships, have excellent communication and organizational skills, and be comfortable in a leadership/training role. On a company wide level we hope you can help to support Elder Care Homecare’s growth, mission, and goals of Growing Stronger Together.

Responsibilities:
- Overseeing Clientele that is assigned to your care
- Creating and maintaining relationships with our Caregivers and Nurses caring for your client
- Executing, implementing, and updating the Plan of Care for the client, ensuring the Caregiver is aware of changes
- Home visits - 75% of your time will be spent out in the field
- Onboarding prospect clients
- Monitoring Discharge from Hospital, SNF, or Rehab
- Completing administrative paperwork in accordance with our policy and NYS DOH
What We're Looking For:
- NYS Registered Nurse with Up to Date License
- Ability to work autonomously
- Ability to work some weekends throughout the month
- Must feel comfortable clinically training RNs and Caregivers on-site to their respective duties for their clients
