The Opportunity
We are seeking a Social Work Care Manager to join our growing team. This role is based in our Westbury office and supports clients across the Western Nassau and Queens area. It is ideal for someone who thrives in a fast-paced, mission-driven environment and wants to be part of an organization that truly values its people.
As a Social Work Care Manager, you will oversee overall case management within clients’ homes. This includes coaching caregivers, recommending changes to care plans, conducting home visits, and acting as a key liaison between clients, families, and internal teams.

Key Responsibilities
- Oversee and manage assigned client cases within the home setting
- Build and maintain strong relationships with caregivers, nurses, clients, and families
- Assist with developing, implementing, and updating client care plans
- Ensure caregivers are informed of and aligned with care plan changes
- Support caregiver-client matching in partnership with the coordination team
- Conduct home visits for supervision, quality assurance, and conflict resolution
- Onboard prospective clients and support care transitions
- Monitor client discharges from hospitals, skilled nursing facilities (SNFs), or rehab centers
- Collaborate with executive leadership and cross-functional teams
- Complete administrative documentation in compliance with company policies and Department of Health requirements
- Occasionally host virtual enrichment sessions for caregivers
Qualifications
- Bachelor’s degree in Social Work or substantial related experience
- 3–5 years of experience in social work and/or care management
- Experience working with older adults, elderly, and geriatric populations
- Strong leadership capability and comfort guiding caregivers and influencing care decisions
- Self-starter with the ability to work autonomously and stay highly organized
- Excellent communication and interpersonal skills with a collaborative mindset
- Strong conflict resolution skills with a solution-oriented approach
- A genuine passion for working with people and supporting families
