Key Responsibilities

• Overseeing Clientele that is assigned to your care
• Creating and maintaining relationships with our Caregivers and Nurses caring for your client
• Executing, implementing, and updating the Plan of Care for the client, ensuring the Caregiver is aware of changes
• Home visits - 75% of your time will be spent out in the field
• Onboarding prospect clients
• Monitoring Discharge from Hospital, SNF, or Rehab
• Completing administrative paperwork in accordance with our policy and NYS DOH

Qualifications

• NYS Registered Nurse with Up to Date License
• Ability to work autonomously
• Ability to work some weekends throughout the month
• Must feel comfortable clinically training RNs and Caregivers on-site to their respective duties for their clients
